SG500-52P - why so many faults?

12 Cisco SG500-52P switches on SW 1.4.5.02 and boot 1.4.0.02. These are all over the offices, some in stacks, some not.

One stack does this all the time. Switch is now unusable from web or console.

Another stack has 4 switches and there are 42 ports out of the 192 that show this:

Every single switch shows faults....even on ports where nothing is plugged in. I can't have every switch replaced. That is insanity.

actually "fault" in that output "show power inline" doesn't mean software or hardware failure. "fault" here means, that switch is unable to detect any powered device connected on that port, which means that no compatible (either IEEE 802.3af PoE or IEEE 802.3at PoE+) device is connected to that particular switchport. but switch is regulairly searching for compatible device which results that status "fault" is should be changing with "searching".
